I didn't do well in chemistry and 100% sure that I will need to fill improvement exams, please help me to fill forms and tell me about the last date.

Forms for the improvement exams are yet to come out. Generally CBSE releases the forms in the 2nd week of august and will be available at regional CBSE offices and syndicate banks branches. Keep an eye on this forum, as soon as forms get announced I will update you regarding the same.
