I once saw a woman fall out of her car while on the entrance ramp to an underground lot. She was stopped at the ticket dispenser but couldn't quite reach the ticket through her open window. She undid her seat belt to get a little extra reach but it still wasn't enough so she opened the door a just a bit. Success at last! Just as she grabbed the ticket her weight pushing on the door caused it to open completely and out she fell. Of course she neglected to put her car in park during this procedure so when her foot came off the brake the car started forward.
