birth certificate, social security card, identification, passports, tax records (only if you think they'll audit you)

Shot history would be good, your family doctor you've been seeing should be enough as whatever doc you go to should cross-ref. However, for a dentist they won't so if you can get your last complete set of x-rays that would be helpful.

Proof that you are elgible to work in the US is helpful too. Passport or Social Security Card.

Proof of insurance, health and auto.

Checkbook as well as bank statements.



uhm...? Put all of that in an envelope and send it to me, I'll look after it for you. Shot history is kinda more because I am in the military and all. I presume with the dentist I can just keep a business card from my old dentist and they can just fax any new information. I just got my wisdom teeth out a month ago so I'll need new xrays next time I will go anyway.

Social Security card is a given. I have that at school with me but in a different place than I keep all other forms of identification.

Never had a passport actually. I don't even know how to get one.

Yeah, I keep the insurance card in my car. I double that as registration too.

Though I do vaguelly remember watching a few happy days reruns as a kid. I never cared for the show much. It seemed horribly outdated. Yes I know it was supposed to take place in the 50's.
