It's a sewer pipe, not a full sewer. A human couldn't fit in there. That's why the camera was put down there on a rail or track. It's probably no bigger than a foot in diameter.
As the camera view is about the middle, it could be as small as 6" in diameter - we often use similar CCTV cameras to check pipes before and after working on them.
They're like long skinny remote controlled vehicles with one or more steerable cameras on them.
